- Cremation authorization forms
- Affidavits
- Permission forms
- General personal documents requiring notarization
Please note that some documents may require specific requirements or witnesses. We recommend calling ahead so we can confirm how we may assist you.
What to Bring
To complete a notarization, please bring:
- A valid government-issued photo ID
- The unsigned document requiring notarization
- Any required witnesses, if applicable
Please do not sign the document before your appointment. The signature must be completed in front of the notary.
